How can I ask my doctor medical questions or make medical requests?

 

We have tried to provide answers for the most commonly asked administrative questions online in our FAQ. If you need further assistance you may give us a call at (905) 884-0666 during our clinic hours.  

 

If you have a question for your physician you may book a phone consultation or leave a message with them.  Please be advised that it may take 3-4 days to respond.  If you need something addressed or answered more urgently please book an appointment with your physician or come as a walk-in. 

 

Please note, results cannot be given out by the receptionists.  If you would like to review test results with your physician please book a follow up appointment either in person or as a phone consult.

How can I request a copy of my medical records?

 

We are sorry to see you go.  To request medical records be transferred to your new physician you will need to sign a Records Transfer Request at their office.  They will then send it to us requesting the records they need.  The Ministry of Health does not cover the cost of records transfers, so a bill will be sent to you based on what is being requested.  Once paid the records will be forwarded.

How long are my records kept?

 

Records are kept for 10 years.  After the record retention period has passed records are shredded to maintain patient confidentiality.

How can I restrict access to my medical records?

 

At All About Health we value the trust you put in us, and this extends to protecting your privacy. Your medical records and personal information are only seen by the doctors and staff members who provide your health care and who do administrative tasks to support your health care, and only when they need to see that information to do their job. Your information is only ever shared with your explicit and express permission, with the exception of a few circumstances whereby we are legally required.

 

Ontario's Personal Health Information Protection Act (PHIPA) guarantees that your health information and personal details are securely stored and that access to them is always restricted. PHIPA also gives you the right to specify how and who has access to your medical records. Where do I go to get blood/imaging tests done?

 

We have listed some of the labs near us in our patient resources for your convenience.  

 

You can use any requisition for bloodwork, x-rays, or ultrasounds at any location.  You are not limited to the locations on your requisition, should another location be more convenient your requisition can be used at any clinic that offers the requested service.

I changed my address/phone number/email address, how do I notify the clinic?

 

Please call our clinic at (905) 884-0666 during our regular business hours in order to update any of your contact information. Please note, updating the Ministry of Health through Service Ontario of your address change does not automatically update our records and vise-versa.

My OHIP is expired or about to expire - what should I do?

 

If your OHIP is expired or due to be expiring soon please visit this link for information on how to renew your OHIP card.  

 

If you require an appointment but have not renewed your OHIP coverage we may have to invoice you the cost of the appointment.

I have renewed my OHIP card - how do I notify the clinic of the updated information?
